All physicals and accompanying paper work will be submitted online through our medical record system ATS. You can access the athlete portal via Etown2.atsusers.com. NO Paper forms will be accepted.
 
If you had an account created last year, you will login with your student ID number and your previous password. If you are a first-year or transfer student you will receive an email  with your ID and temporary password as well as instructions on how to complete your athletic's  and student-health paperwork/ forms.

You will need to print out the physical paperwork below and take it with you to your doctor's appointment. Once that is signed you will upload it to the new medical record system.  If you are on medication for ADHD, you will also need to print out and take that paper with you to the doctor's appointment and once again upload it to ATS.
 
IMPORTANT DATES
June 1 - First day you can receive a physical for the upcoming academic year.
August 1 - ALL physical paper work needs to be completed and submitted through ATS.
